We understand the challenges that you may face so by using a biopsychosocial model for service delivery, iMove Rehabilitation will prescribe efficient, simple, functional, personalised exercise programs that are tailored to the individuals’ disease and condition.
After receiving the referral from the GP or specialist, iMove Rehabilitation will contact you and organise an initial assessment.
Following the initial assessment, you will have a treatment plan developed that will see you embark on a safe and effective exercise program tailored specifically for your needs while ensuring all key stakeholders are continuously in communication during the process.
Regular exercise and physical activity provide a range of physical, mental and psychosocial benefits. The research, clinical and anecdotal evidence supports Exercise is Medicine!
Our Exercise Physiology Services are Bulk Billed when you are referred by your GP with one of the following referrals:
- Enhanced Primary Care Plan / Chronic Disease Management Plans
- GP Management Plans for patients with type II diabetes – group
- DVA patients with a D904 referral
